If you’ve just been diagnosed, or are supporting someone who has, the most important thing to know is this: HIV is now a manageable long-term condition, not the illness it was a generation ago. Treatment is effective, usually simple, and lets people live full lives.

What ART Does

How HIV treatment works

Antiretroviral therapy (ART) is a combination of medicines — most often a single daily tablet — that stops HIV from multiplying. Over weeks to months it brings the amount of virus in the blood (the viral load) down to undetectable levels, lets the immune system recover, and keeps it there for as long as treatment continues. Modern regimens are well-tolerated, and any side effects are usually mild and manageable.

After Diagnosis

What happens after a positive or reactive HIV test?

A reactive screening test isn’t the end of the story. The next steps are to confirm the diagnosis, assess your immune system, and start treatment safely.

1

Confirm the result

A reactive screening test is confirmed with appropriate HIV confirmatory testing before long-term treatment decisions are made.

2

Baseline assessment

Blood tests check viral load, CD4 count, kidney/liver safety, hepatitis status, TB risk and other infections.

3

Start ART early

Modern HIV treatment is usually started early, once the diagnosis and baseline assessment are complete.

4

Monitor and support

Follow-up checks confirm the viral load is falling, side effects are controlled and treatment is working.

Baseline Workup

Tests usually needed before or soon after starting ART

Baseline tests help choose the right ART regimen, assess safety, and find infections that need attention.

PurposeCommon tests
Confirm HIV & disease statusConfirmatory HIV test, HIV viral load, CD4 count
Medicine selectionHIV drug-resistance testing where indicated/available
Medicine safetyCBC, kidney function, liver function, glucose/HbA1c, lipid profile
CoinfectionsHepatitis B & C, syphilis and other STI screening where relevant
TB & opportunistic infectionsTB screening and others based on symptoms, CD4 and clinical risk
Pregnancy / family planningPregnancy test and reproductive counselling where relevant
Viral Suppression

How long does ART take to work?

Once ART is started and taken consistently, the viral load usually begins falling quickly. Many people reach an undetectable viral load within a few months, though timing varies with baseline viral load, adherence and regimen.

U=U depends on maintaining an undetectable viral load — it’s confirmed through viral load monitoring, not simply by starting tablets.
U=U — One of the Most Important Facts in HIV

Undetectable = Untransmittable

U=U means Undetectable = Untransmittable. A person on effective ART whose viral load is undetectable cannot transmit HIV to sexual partners. This is settled science, and it changes everything — for relationships, for planning a family, and for the stigma that used to surround HIV.

It’s also a powerful reason to test and start treatment early: the sooner the virus is suppressed, the sooner both your health and your peace of mind are protected.
Important: U=U applies when the viral load is confirmed undetectable and stays undetectable on treatment. It does not mean HIV is cured, and treatment should not be stopped.
Living Well with HIV

What ongoing care looks like

Care settles into a simple rhythm: take the medicine consistently, with periodic check-ins to confirm the viral load stays undetectable and the rest of your health stays well. People on treatment work, travel, have relationships and have children safely. The two things that matter most are starting and staying consistent — everything else is supported.

Staying Consistent

Why adherence matters

ART works best taken consistently. Repeatedly missing doses can let the viral load rise again and may increase the risk of drug resistance.

  • Don’t stop ART on your own.
  • If you miss a dose, ask your doctor how to handle it for your regimen.
  • If side effects, cost or stigma make adherence hard, raise it early — the regimen can often be adjusted.
  • Regular viral load monitoring confirms treatment is working.
Safety

When should you contact your doctor urgently?

  • Fever, weight loss, persistent diarrhoea, cough, breathlessness or severe weakness
  • Severe rash, jaundice, vomiting or a suspected medicine reaction
  • New neurological symptoms, severe headache or confusion
  • Pregnancy or planning pregnancy while on ART
  • Repeated missed doses or stopping ART

Frequently Asked Questions

Common Questions

Can HIV be cured?
There is no cure yet, but HIV is very effectively controlled with antiretroviral therapy (ART) — usually a single daily pill that brings the virus to undetectable levels and allows a long, healthy life. Treatment is ongoing rather than curative.
What is U=U?
U=U means Undetectable = Untransmittable: a person on effective HIV treatment whose viral load is undetectable cannot transmit HIV to sexual partners. It’s established science and one of the strongest reasons to test and treat early.
What is life expectancy with HIV today?
With consistent modern treatment, people with HIV can expect a near-normal life expectancy. The key is starting ART and taking it consistently, with periodic monitoring.
Is HIV treatment lifelong?
Yes — ART is taken long-term to keep the virus suppressed; stopping allows it to rebound. Fortunately, modern treatment is usually one well-tolerated pill a day, and care is straightforward.
Can someone with HIV have children safely?
Yes. With effective treatment and an undetectable viral load (U=U), and appropriate medical guidance, people with HIV can have relationships and children safely. A specialist can advise on planning.
When should HIV treatment be started?
Usually as early as possible after diagnosis and baseline evaluation. Early treatment protects the immune system, lowers viral load, and helps prevent transmission once the viral load is undetectable.
How long does it take to become undetectable?
Many people become undetectable within a few months of starting ART, though timing varies. Viral load testing confirms whether treatment is working and whether U=U applies.
Can I stop ART after my viral load becomes undetectable?
No. Undetectable does not mean cured — ART must continue to keep the virus suppressed. Stopping treatment can let the viral load rebound.
What tests are monitored during HIV treatment?
Common monitoring includes HIV viral load, CD4 count when needed, kidney and liver function, blood counts, metabolic parameters, and screening for coinfections depending on the situation.
Can my partner stay HIV-negative?
Yes. Effective ART with an undetectable viral load prevents sexual transmission of HIV. Partner testing, condoms and PrEP may also be discussed depending on timing and situation.
