The NRI Diabetes Problem

South Asians are genetically predisposed to insulin resistance and Type 2 diabetes — yet most NRIs receive fragmented, reactive care that manages blood sugar without ever targeting remission.

3-Month Wait Times

Endocrinologists in the US, UK, and UAE often have 3–6 month waiting lists. Your quarterly HbA1c check becomes a biannual event. Metabolic momentum is lost.

🔬

HbA1c Is Not Enough

A quarterly HbA1c gives you a 3-month average — it misses the daily spikes, nocturnal lows, and post-meal surges that drive complications. You need CGM-level granularity.

🧬

South Asian Phenotype Ignored

Western endocrinologists often apply generic diabetes protocols. South Asians develop insulin resistance at lower BMIs and respond differently to medications. This nuance matters.

💊

GLP-1 Without Stewardship

Ozempic and Mounjaro are transformative — but many NRIs are on them without structured monitoring. Dose titration, side effect management, and remission criteria need specialist oversight.

Beyond HbA1c — The CGM Revolution

Modern diabetology has moved from reactive blood-sugar management to proactive metabolic control using Continuous Glucose Monitoring. Dr. Manuj analyses your CGM data to see what your HbA1c hides.

Time in Range, glycemic variability, nocturnal patterns, post-meal spikes, dawn phenomenon — these metrics tell the real story of your metabolic health. One CGM review often achieves more dietary change than six months of counselling.

What the Metabolic Health Desk Includes

Every quarter, you get a comprehensive metabolic review — not a 10-minute appointment, but a deep-dive into your data by a specialist who knows your history.

CGM Data Review

Upload your FreeStyle Libre, Dexcom, or Guardian data. Dr. Manuj analyses Time in Range, glycemic variability, nocturnal patterns, and post-meal responses.

HbA1c Trajectory Tracking

Your HbA1c plotted over quarters — not just the number, but the trend, the rate of change, and what it means for your remission timeline.

Medication Optimisation

Review of all diabetes medications — metformin, sulfonylureas, SGLT2 inhibitors, insulin — for dosing, timing, interactions, and whether they are still necessary.

GLP-1 Stewardship

If you are on Ozempic, Mounjaro, or Rybelsus: structured dose titration, side effect management, and remission criteria assessment. If you are not: whether you should be.

Quarterly Video Consultation

30–45 minute video call with Dr. Manuj. Timezone-friendly scheduling. Discussion of results, treatment adjustments, and goals for the next quarter.

Between-Visit WhatsApp Access

Questions about a new food spike? Side effect concern? Message Dr. Manuj's clinic between quarterly reviews. No waiting for the next appointment.

Structured Quarterly Report

A detailed metabolic review document you can share with your local doctor. Includes CGM interpretation, medication recommendations, and risk stratification.

Remission Roadmap

Not just management — a structured path toward diabetes remission where clinically possible. Clear milestones, lifestyle targets, and medication tapering criteria.

The Quarterly Subscription

One subscription. Continuous metabolic oversight. Cancel or pause anytime.

GLP-1 Medications We Supervise

Dr. Manuj has deep expertise in all three major GLP-1 agonists — understanding availability, cost, and naming differences across the USA, UK, UAE, and Australia.

Ozempic
Semaglutide (injectable)

Weekly injection. Gold standard for T2DM with weight loss benefit. Dose titration critical to minimise GI side effects.

Mounjaro
Tirzepatide (injectable)

Dual GIP/GLP-1 agonist. Superior A1c reduction in trials. Newer agent requiring careful titration and monitoring.

Rybelsus
Semaglutide (oral)

Oral GLP-1 for needle-averse patients. Requires strict fasting protocol. Absorption-sensitive — timing guidance is essential.

Trulicity
Dulaglutide (injectable)

Weekly injection with simpler dosing. Good option when semaglutide is unavailable or cost-prohibitive in your country.

Who Is This For?

The Metabolic Health Desk is designed for NRIs who want more than reactive diabetes management — they want a path to remission.

📊

NRIs on CGM

You wear a FreeStyle Libre or Dexcom but your local GP doesn't do deep CGM interpretation. You want a specialist who reads the patterns, not just the average.

💊

NRIs on GLP-1 Therapy

You're on Ozempic or Mounjaro but without structured monitoring. Nobody is tracking whether you're actually heading toward remission or just managing.

📈

Stuck HbA1c

Your HbA1c has been hovering at 7–8% for years despite treatment. Something is being missed — and a fresh set of MRCP UK-trained eyes can find it.

🧬

South Asian Heritage

You want a diabetologist who understands that South Asians develop insulin resistance at lower BMIs and respond differently to Western-standard protocols.

Fed Up With Wait Times

Your endocrinologist appointment is 4 months away. You want a detailed metabolic review within days, not quarters.

🎯

Want Remission, Not Just Management

You've been told "diabetes is for life." You want a second opinion from a specialist who has helped patients achieve measurable remission.

Frequently Asked Questions

Do I need to wear a CGM to join?
No. If you wear a CGM (FreeStyle Libre, Dexcom, Guardian), we provide deep data analysis. If you don't, Dr. Manuj works with your HbA1c, fasting glucose, post-meal readings, and other available data. He may recommend a 2-week CGM trial if clinically indicated — these are available in most countries.
Can Dr. Manuj prescribe GLP-1 medications for me?
The Metabolic Health Desk is an advisory service. Dr. Manuj provides detailed prescribing recommendations — specific medication, dosage, titration schedule, and monitoring plan — that your local doctor can implement. For patients within India, Dr. Manuj can prescribe directly.
I'm already seeing an endocrinologist. Why do I need this?
This isn't a replacement — it's a specialist layer. Many endocrinologists see patients for 10–15 minutes every 3–6 months. The Metabolic Health Desk gives you a 45-minute deep-dive, CGM-level analysis, between-visit access, and a remission-focused perspective that standard appointments rarely provide.
What is diabetes remission — is it really possible?
Diabetes remission means achieving and maintaining HbA1c below 6.5% without glucose-lowering medications for at least 3 months. It is achievable for many Type 2 patients through a combination of weight loss, dietary changes, exercise, and strategic use of medications like GLP-1 agonists. Not every patient will achieve full remission, but most can achieve significant metabolic improvement.
How is this different from a one-time second opinion?
A second opinion is a snapshot — one review, one report. The Metabolic Health Desk is an ongoing relationship. Dr. Manuj tracks your metabolic trajectory over quarters, adjusts recommendations based on how your body responds, and provides continuous guidance. Remission is a process, not a single event.
Can I cancel or pause my subscription?
Yes. You can cancel or pause anytime. There are no lock-in contracts. Each quarter is billed independently. If you pause and resume later, Dr. Manuj picks up where you left off — your complete metabolic history is maintained.
What does Dr. Manuj know about medications available in my country?
Dr. Manuj maintains current knowledge of drug availability, naming conventions, and cost structures across the US, UK, UAE, Canada, and Australia. For example, he knows that Mounjaro availability differs between the US and Australia, that Ozempic costs vary dramatically with insurance coverage, and that some formulations available in India are not approved in certain Western markets. Recommendations are always country-specific.
