Cough That Won't Go Away? Causes & When to See a Doctor

A cough lasting more than about 2–3 weeks should be checked. Common causes include post-viral airway sensitivity, allergies or asthma, acid reflux, post-nasal drip, and chest infections — and, in India, tuberculosis always needs to be considered. Dr. Manuj Sondhi, MRCP (UK), a Consultant Physician at Nirvana Clinic, Greater Noida, evaluates persistent cough properly rather than just repeating cough syrups.

When a Cough Needs Prompt Attention

⚠ Seek prompt medical care if your cough comes with:

  • Coughing up blood
  • Breathlessness or chest pain
  • High fever, or fever with night sweats and weight loss (possible TB)
  • A cough lasting more than 3 weeks that isn't improving
  • Severe difficulty breathing or blue lips — this is an emergency
Common Causes

Why a Cough Can Linger for Weeks

Most long-lasting coughs are not serious, but a few causes are important to rule out. A physician's job is to tell which is which rather than treating blindly.

Post-viral cough

After a cold or flu, the airways can stay irritated for weeks. This is common and usually settles — but should be reviewed if it drags on.

Asthma & allergy

Allergic or asthmatic airways cause a dry, often night-time or exercise-triggered cough that responds to specific treatment, not antibiotics.

Acid reflux (GERD)

Reflux is a frequently missed cause of chronic cough, often worse when lying down or after meals.

Post-nasal drip / sinusitis

Mucus dripping from the nose or sinuses into the throat is a very common trigger for a persistent cough.

Chest infection

Bronchitis or pneumonia can cause a productive cough with fever. An infection assessment may be needed.

Tuberculosis (TB)

In India, a cough beyond 2–3 weeks — especially with weight loss, fever or night sweats — must be tested for TB. This is important and treatable.

BP medicine side-effect

A class of blood-pressure medicines (ACE inhibitors) commonly causes a persistent dry cough. A medicine review can confirm this.

Smoking & airway disease

Smoking-related airway disease (COPD) is a cause of long-term cough that benefits from proper assessment.

How a Physician Helps

1
Detailed historyDuration, dry vs productive, triggers, fever, reflux, smoking and current medicines — this narrows the cause quickly.

Examination & targeted testsChest examination, and where indicated a chest X-ray, TB testing or lung-function test — ordered for a clear reason, not routinely.
3
Cause-specific treatmentTreatment aimed at the actual cause — reflux, allergy, infection or medication change — rather than repeated cough syrups.
4
Safe follow-upA clear plan for when to review and what warning signs mean you should return sooner.
Common Questions

Persistent Cough — FAQs

How long is a cough "too long"?
A cough lasting more than about 2–3 weeks, or one that keeps coming back, should be reviewed by a doctor. Most are not serious, but a few important causes need to be excluded.
When should I worry about TB?
In India, a cough lasting more than 2–3 weeks — especially with fever, night sweats, weight loss or coughing blood — should be tested for tuberculosis. TB is common and very treatable when diagnosed.
Can blood-pressure medicine cause a cough?
Yes. A class of BP medicines called ACE inhibitors commonly causes a persistent dry cough. If you started a new BP medicine before the cough began, a medicine review can check whether it is the cause.
Do I need a chest X-ray for a cough?
Not always. A chest X-ray or other test is ordered when the history or examination points to a reason for it — for example a long-lasting cough, fever, breathlessness or red-flag symptoms — rather than for every cough.
