Insulin Resistance: The Hidden Stage Before Diabetes — Testing & Treatment in Greater Noida

Years before blood sugar rises, the body can quietly become resistant to its own insulin — driving belly fat, fatigue after meals, dark neck patches, PCOS and fatty liver. Dr. Manuj Sondhi (MRCP (UK), MD, DNB), Consultant Physician & Diabetologist at Nirvana Clinic, Greater Noida, tests for insulin resistance directly and treats it before it becomes diabetes.

Quick answer: Insulin resistance means your muscles, liver and fat cells respond poorly to insulin, so the pancreas pumps out extra insulin to keep sugar normal. Standard sugar tests can stay normal for years while this is happening. It is detected with fasting insulin, HOMA-IR, HbA1c and body-composition assessment — and in many people it can be improved substantially with structured lifestyle change and, where indicated, medicines, under a doctor’s supervision.

Signs Your Body May Be Insulin Resistant

None of these confirms the diagnosis on its own, but the more that apply, the stronger the case for testing:

Visible signs

  • Increasing waistline or belly fat despite a normal-looking weight
  • Dark, velvety patches on the neck, armpits or knuckles (acanthosis nigricans)
  • Skin tags around the neck or underarms

How you feel

  • Heavy sleepiness after meals, especially rice or sweets
  • Frequent hunger or sugar cravings soon after eating
  • Low energy despite adequate sleep

In your reports & history

  • Borderline sugar, HbA1c 5.7–6.4%, or “prediabetes” remarks
  • High triglycerides with low HDL cholesterol
  • Fatty liver on ultrasound; PCOS; high blood pressure
  • Parent or sibling with type 2 diabetes

Indian adults develop insulin resistance at lower body weights than Western populations — which is why a “normal BMI” with a growing waist still deserves testing. Related reading: what your HbA1c number actually means.

How We Test for Insulin Resistance at Nirvana Clinic

TestWhat it showsPreparation
Fasting insulin + fasting glucoseThe raw ingredients of the HOMA-IR calculation; high fasting insulin with normal sugar is the classic early signature.8–12 hours fasting
HOMA-IR scoreA calculated index of insulin resistance from fasting insulin and glucose; interpreted by the doctor in your clinical context, not as a standalone verdict.Same sample as above
HbA1cYour 3-month average sugar — detects prediabetes (5.7–6.4%) and diabetes (≥6.5%).No fasting needed
Lipid profileHigh triglyceride-to-HDL pattern is a common companion of insulin resistance.Fasting preferred
Liver enzymes ± ultrasoundScreens for fatty liver, which both results from and worsens insulin resistance. As advised
InBody 380 body compositionMeasures visceral fat level and skeletal muscle mass — the two body compartments that drive and fight insulin resistance respectively.In-clinic, under a minute

Test selection is individualised — not everyone needs every test. Reference ranges and interpretation vary by lab and clinical context; results are always reviewed with the doctor.

How Insulin Resistance Is Treated

1 · Lifestyle — the foundation

Structured changes to meal composition (protein and fibre first, refined carbohydrates down), meal timing, daily movement, resistance exercise to build muscle, sleep and stress. This is prescribed specifically, not as vague “eat healthy” advice.

2 · Treating the drivers

Visceral fat, fatty liver, PCOS, thyroid problems and certain medicines all worsen insulin sensitivity. Each identified driver gets its own plan — for example, our fatty liver programme or PCOD & metabolic syndrome care.

3 · Medicines — where indicated

In selected patients, medicines such as metformin, and in eligible cases GLP-1 based therapy, are added to lifestyle treatment. All are prescription-only decisions made after testing — never a default.

4 · Measure, don’t guess

Repeat testing and InBody 380 monitoring track whether visceral fat is falling and muscle is holding — the objective proof that insulin sensitivity is improving.

Response varies between individuals. Improvement in insulin resistance is common with sustained treatment, but the degree and pace differ from person to person, and some patients will still progress to diabetes despite good care — early treatment improves the odds, it does not promise an outcome.

Frequently Asked Questions

What is insulin resistance in simple words?

Insulin is the hormone that moves sugar from your blood into your cells. In insulin resistance, the cells respond poorly, so the pancreas produces extra insulin to compensate. Sugar levels can stay normal for years while insulin runs high — which is why the condition is often missed until prediabetes or diabetes appears.

Can I have insulin resistance if my sugar reports are normal?

Yes, and this is very common. In the early years, high insulin keeps glucose readings normal, so a routine sugar test looks fine. Fasting insulin and the HOMA-IR index can detect the problem at this stage, when it responds well to treatment.

Which test confirms insulin resistance?

There is no single perfect test. Doctors combine fasting insulin, fasting glucose and the calculated HOMA-IR index with HbA1c, lipid profile, liver assessment and body composition, interpreted in your clinical context. A raised HOMA-IR with typical clinical features usually establishes the picture.

Is insulin resistance the same as prediabetes?

No. Insulin resistance is the underlying mechanism; prediabetes is a later stage where sugar has started rising into the HbA1c 5.7 to 6.4 percent range. Many people have insulin resistance for years before reaching prediabetes — treating it at this earlier stage is the ideal window for prevention.

Can insulin resistance be reversed?

Insulin sensitivity can improve substantially in many people with sustained weight and visceral fat reduction, muscle building, dietary change and treatment of drivers like fatty liver. The degree of improvement varies between individuals, and maintaining it requires maintaining the habits — so doctors prefer the word improvement over a permanent cure.

Why do Indians get insulin resistance at lower body weights?

South Asian body composition tends toward more visceral fat and less muscle mass at the same BMI compared with Western populations — sometimes called the thin-fat phenotype. This is why Indian guidelines use lower BMI and waist cut-offs, and why a normal-weight person with a growing waistline still deserves testing.

Does insulin resistance cause PCOS and fatty liver?

Insulin resistance is a central driver of both. High insulin levels stimulate the ovaries and disturb hormone balance in PCOS, and push the liver to store fat. Treating insulin resistance is therefore part of the core treatment of both conditions, alongside their specific care.
