Important to know: Not all memory loss is dementia. What a Memory Evaluation Involves at Nirvana Clinic

1. Detailed history — from the person and, importantly, a family member. When the changes started, how fast they are progressing, and how daily life is affected.

2. Cognitive assessment — standardised, structured tests of memory, attention, language, and orientation performed in the clinic. These are painless, take under an hour, and give an objective baseline to track over time.

3. Ruling out treatable causes — this step is where our two-specialist clinic makes a real difference. Blood tests for vitamin B12 deficiency, thyroid function, blood sugar, and other medical contributors are coordinated with Dr. Manuj Sondhi, Consultant Physician, under the same roof. Uncontrolled diabetes and hypertension also affect brain health and are reviewed as part of the plan.

4. Screening for depression and sleep problems — depression in older adults can look exactly like dementia ("pseudodementia") and often improves significantly with treatment. Sleep disorders also impair memory and are assessed.

5. Brain imaging where indicated — an MRI or CT scan may be advised in selected cases; we guide you on where and when, and interpret the report in context.

6. Diagnosis, honestly explained — if dementia is diagnosed, the type, stage, and what to expect are explained to the family clearly and compassionately, in English, Hindi, or Bengali.

How Dementia Is Managed

Dementia currently has no cure, and we will never tell you otherwise. What honest, evidence-based care can do is meaningful:

• Medicines that may help stabilise symptoms in suitable patients, prescribed and reviewed carefully
• Treatment of agitation, sleep disturbance, suspicion, or low mood — the symptoms that most exhaust families
• Management of medical conditions (BP, sugar, B12, thyroid) that can worsen cognition
• Practical caregiver guidance: daily routines, safety at home, communication techniques, and planning ahead
• Regular follow-up so changes are caught early and the plan evolves with the illness

Sudden confusion is different. If an elderly person becomes acutely confused, drowsy, or disoriented over hours to days — especially with fever, dehydration, or after a fall or new medicine — this may be delirium, a medical emergency. Can dementia be cured or reversed?
True dementia (such as Alzheimer's disease) cannot currently be cured or reversed, and any claim otherwise should be treated with caution. However, some conditions that mimic dementia — vitamin B12 deficiency, thyroid imbalance, depression, and medicine side effects — are treatable, which is why evaluation before diagnosis is essential.
At what age should memory problems be taken seriously?
At any age, if memory problems interfere with daily functioning or are noticed to be progressing by family members. While dementia is more common after 65, evaluation is appropriate whenever the pattern looks beyond normal ageing — earlier assessment gives a clearer baseline and more options.
My father refuses to see a "mental doctor." What can we do?

What should we bring to the first appointment?
All current medicines (or a photo of them), previous prescriptions and test reports, and one family member who knows the person's daily routine well. A short written note of specific incidents — what was forgotten, when — is extremely helpful.

Yes. Agitation, sleep disturbance, suspicion, and repetitive behaviours are among the most distressing symptoms for families, and they are a core focus of psychiatric management — through environmental and caregiving strategies first, and carefully monitored medicines where needed.
